*$7,500 sign-on bonus: $3,750 after 30 days and $3,750 after 6 months.

Key Responsibilities:
- Responsible to work with the Assisted Living Manager to achieve effective management of the Assisted Living and coordinate with other ancillary health providers to provide quality care and services to the residents.
- Assigns and directs the work of caregivers on their respective shifts and will assist in annual evaluations for the caregivers.
- Projects a positive image to fellow staff, residents, families, and physicians.
- Records and is responsible for carrying out physicians' orders in a timely manner; ensure medications are administered per physician orders.
- Ongoing evaluation of residents and condition while performing routine treatments and resident care; and notifies proper individuals.
- Maintains records in accordance with community policy. This includes, but is not limited to, documentation in charts and maintenance of medication records (controlled and uncontrolled).
- Will assist in orientation of new employees and will be responsible for the input in the evaluation of the new employees.
- Administers treatments and medications within the scope of Arizona Regulations.
- Initiates, updates, and participates in Resident Service Plan.
- Tracks and reports Quality Measures.
- Participates in all the daily activity of the assisted living residents.
- Works with the Director of Dining Services to maintain an assisted living menu including snacks.
- Cooperates with all other departments in providing a favorable physical, social, and emotional environment for all residents.
- Assists with ordering supplies for the care staff to meet their job requirements.
- Ensures that resident care interventions meet the personal, physical, and cognitive needs of each resident as well as maximize his/her self-care capacities, identity, independence, choice, and opportunity for social interaction.
- Visits resident care areas daily and observes the care services that the residents are receiving to ensure quality care.
- Monitors and reviews resident incidents, updates service plans with interventions, and communicates the plan of care to the care team.
- Ensures that all care staff adhere to the infection control practices.
- Required tuberculosis monitoring is accomplished, documented, and filed.
- Respond to emergency needs as designated by community policies.

Your Qualifications:
- Graduate of an accredited school of nursing.
- Must possess a current, active license to practice as a LPN in Arizona
- Must possess and maintain a current adult CPR certificate
- Attends continuing education programs required for maintenance of professional certifications and licensure
